How to make your appointment

To set up an appointment, just call Summit Medical Associates at (404) 607-0042 / (800) 537-2985 between 8:00 am and 5:00 pm Monday through Friday, or 8:00 am and 2:00 PM on Saturdays. Of course, you're welcome - and even encouraged - to call us with any questions you may have prior to actually scheduling your procedure.

Instructions for your appointment

1. Do not eat, drink or smoke anything as instructed when making your appointment. This includes candy, gum and water.

2. Leave all valuables, such as your watch and jewelry, at home.

3. Plan to be here 4-6 hours. This allows time for your preliminary examination, lab work, personal and financial counseling, the procedure itself and recovery.

4. You must have someone else drive you home.

5. There are a number of ways to pay for your procedure. You may use cash, MasterCard or Visa at the time of your appointment. No personal checks. If you have insurance coverage, please call our financial counselor to make arrangements in advance.

6. If you wear contact lenses, please bring your case and solution.

7. Minors (17 and younger) must be accompanied by a parent or legal guardian. We will need a copy of birth certificate and picture ID from both patient and parent.
